1. Who is eligible to participate in the POLA Wellness event? 

Wellness Week is open to Port of Los Angeles (“POLA”) staff only. POLA staff is defined as employees, student workers, consultants, and contractors. 

2. When does the event take place?

The Wellness Week begins 9:00 AM Pacific Time (“PT”) on October 21, 2024 and ends 8:00 PM PT on October 25, 2024.  

3. How do I participate in the event?

During the Program Period, participate in available event Activities. With your phone, scan the QR code inside the Rewards Passport to be directed to www.POLAWellness.org for the Event Schedule and to sign up for Activities. 

4. How do I earn points for prizes? 

RAFFLE TICKETS: Raffle tickets can be obtained in two ways:  

  • Blue Raffle Tickets were provided through the Wellness Packet sent to all employees. 

  • Other Raffle Tickets can be earned after participating in in-person event Activities during the Health Fair. An event facilitator will distribute tickets at the end of each activity for a chance to win a prize while supplies last. 

Write your full name, division, and phone number on the raffle tickets, which will be entered into a drawing for prizes. Submit raffle tickets any time during the Program Period at any in-person Event Activity, at designated locations during the Health Fair, or at the locations identified below:  

  • C&M Administration Building Lobby 

  • Port Police Headquarters Lobby   

  • HAB Lobby   

 

REWARDS PASSPORT: During the Program Period, participate in available event Activities.  

  • In-person Activities: Attendance for at least 75%  or more during the respective in-person activity is required to receive credit. Present your Rewards Passport and an onsite event staff personnel will stamp your Rewards Passport as proof of attendance for points.  

  • Virtual Sessions: Attendance for 100% of a virtual event is required to receive credit. A code will be provided in the virtual session for you to write into the designated event slot on your Rewards Passport.  

Write your full name, division, and phone number on the front of the Rewards Passport, which will be entered into a drawing for prizes. For final tally, please tally your total points at the bottom of the front cover where it lists “TOTAL PTS”. Anytime during or at the end of the Program Period, and in order to be eligible to participate in the drawing for prizes, participants must submit the hard copy of the Rewards Passport to any of the following locations for final calculation.   

  • C&M Administration Building Lobby 

  • Port Police Headquarters Lobby   

  • HAB Lobby   

5. How do I earn points?

Points are given per each In-Person Activity or Virtual Session attended as described above.   

  • Lunch Time Fitness (Gym Days and Walking Days) = 3 points per session / max 12 points   

  • Virtual Session(s) = 2 points per session / max 30 points 

  • Health Fair Workshop(s) (e.g. Stretching Session) = 3 points per session / max 12 points 

  • Health Fair Primary Care Screening(s) (e.g. Spinal Assessment) = 5 points per visit / max 15 points 

  • Health Fair Wellness Service(s) (e.g. Flu Shot) = 2 points per one time visit / max 8 points 

  • Other = 3 points per one time visit / max 6 points 

6. How many total points can I earn during the event?

The maximum number of points you can earn is 83.

7. What prizes are available?

Prizes are divided into different levels. Limit one prize per POLA employee.  

  • Prize level 1 is available to employees, student workers, consultants, and contractors.  

  • Prize levels 2-3 are available to employees only.  

Prizes are awarded "as is," and there are no substitutions or cash redemptions. Prizes must be collected within the time specified or will be forfeited.     

8. Can I combine or transfer my points to someone else?

No, points cannot be combined, transferred, sold, or disposed of. Any attempt to do so will result in disqualification from the Rewards Program. 

9. What happens if I do not turn in my Rewards Passport at the end of the event?

Any Rewards Passport not turned by the end of the event will be voided. Points have no cash value and can only be redeemed for rewards within the Program Period. 

10. How will the winners be announced?

Final Announcement of winners will be on October 30, 2024 via email.   

11. What should I do if I experience technical issues or errors during participation?

If you experience technical difficulties or have any questions, please contact the POLA Wellness team ( for assistance. POLA is not responsible for system malfunctions or network failures. 

12. What is the policy on prize transfers and substitutions?

Prizes are non-transferable and cannot be substituted unless at the discretion of the sponsor. POLA reserves the right to substitute prizes of equal or greater value if the original prize becomes unavailable. 

13. Are there any privacy concerns regarding my participation?

Any personal information collected during the event will be used solely for administering the program and will not be sold or shared with third parties. The event complies with the California Consumer Privacy Act (CCPA). 

14. Can the event be postponed or canceled?

In the event of unforeseen circumstances (such as natural disasters or pandemics), POLA reserves the right to modify, suspend, or cancel the event without notice.
